eCare Information

Pharmacists can use the eCare Information screen to log and track information related to medical issues they are working on with patients. The data can then be submitted to Community Care North Carolina (CCNC) if the pharmacy has signed a Data Sharing Attestation and Authorization form with CCNC.

Note: Must be authorized for the CCNC eCare Plan flag if using to submit to CCNC. This flag cannot be enabled if the Creative Pharmacist flag is enabled.

Access eCare Information from the Patient Chart.

To access:

  1. Click Patient eCare Plan on the vertical toolbar from the Patient Chart.
  2. The Patient eCare Plan Information displays. Enter the patient and insurance Information.

  3. Click Encounter on the vertical toolbar. The Patient eCare Encounter screen displays. Enter encounter information and up to five Medication Therapy Problems (MTPs) per page, up to 20 in all, along with comments. Click the Select medications associated with above MTP to select a prescription to link to the problem. A scan displays with prescriptions to associate. The link changes to Medications associated with above MTP once any prescriptions are chosen.

  4. Click Interventions on the vertical toolbar. The Patient eCare Interventions screen displays. Use the top drop down to input which MTP you are documenting and the corresponding link to associate any medications necessary. Then, enter up to five interventions along with comments and status using the corresponding links and drop-down boxes.

  5. Click Education on the vertical toolbar. The Patient eCare Education screen displays. Select the MTP to document and enter up to five education entries along with comments and status.

  6. Click Goals and Outcomes on the vertical toolbar. The Patient eCare Goals and Outcomes screen displays. Select the MTP to document and enter goals, outcomes and progress.


  7. Click Care Coordination on the vertical toolbar. The Patient eCare Care Coordination screen displays. Select details about coordination, enter recipient information and hand-off comments.

  8. Click Referrals on the vertical toolbar. The Patient eCare Referrals screen displays. Enter up to five referrals, along with details.

  9. Press CTRL+S or click .

To submit to CCNC:

You must be signed up with CCNC to do submissions as well as have the CCNC eCare Plan flag turned on.

  1. Once all information is complete, click Submit on the horizontal toolbar.
  2. The system reviews the Patient Info, Encounter, Care Coordination, Referrals, Interventions and Education screens for any missing fields. A Missing Information window displays noting which screens are missing required information. Populate any of the missing fields that are denoted with an asterisk (*).
  3. The data transmits to CCNC.
