The MED-PAK Form is a legal requirement in certain states for pharmacies using pill box containers such as Mediset (pill boxes which are used four times a day for seven days). This form requires a 'dummy' printer named QS1LASER with a PCL print driver on the QS/1 Server. This supports additional fonts the form uses.
The process of generating a report in NRx involves a set of steps referred to as Report Logic. These steps include selecting data, sorting the selected data in a certain order and then printing the report. Review Reports Overview for details on these steps.
To print this form:
1. Access NRx.
2. Click Reports. Select Prescription and click MED-PAK Form. Select Options display.
3. Complete the Select Options and click Next. Sort Options display.
4. Complete the Sort Options and click Next. Print Options display.
5. Use the following definitions to complete the Print Options:
Restart Name: To restart printing with one particular patient, type that patient name.
Beg/End Date?: Type the beginning and ending date for prescriptions to print. Use the date format mmddyy-mmddyy. If the beginning date is entered as 000000, the original date of the prescription is used and all prescriptions through the ending date print.
Omit Freq: Enter the frequency code of prescriptions you do not want to print.
Prt Blank Frm: Enter YES to print forms with only patient information. Enter NO to print patient and prescription information.
HP Emulation: Enter YES for a laser printer that uses PCL Emulation. Enter NO for a laser printer that uses IBM PPDS Emulation.
Admin Times: Type the time the prescriptions are to be administered. For example, 8AM, 12PM, 4PM and 10PM would be entered as "812410" space812space410.
# of Days: Three position numeric field that defaults to 000. If any other number is entered, the system checks each prescription's SIG Record for Qty/Day. If there are multiple SIGs, the system checks each one and adds the Qty/Day. The number entered for # of Days is then multiplied by the Qty/Day or sum of multiple SIGs. This result prints in the QTY column on the form. If the SIG Qty/Day is 0, then the quantity dispensed from the prescription prints.
Type Codes: Enter one patient non-drug order type code. The first non-drug order prints up to 100 characters below the patient phone number in the bottom section of the form.
Export to File: Enter YES to save as a .txt file on the QS1 Server. The file will be created in the QS/1 folder under the path \AppData\Rx\MEDPAK. One file is created per patient so data may be printed at a later time with the Reprint Form and MED-PAK ID options. The file name is defined with a prefix of MP, Patient MED-PAK ID, and the date the file was created. For example, a file created for patient John Smith on January 1, 2011 would be MPSMITHJOHN01012011.txt.
Reprint Form: Enter YES if reprinting a form. The ID must be entered in the MED-PAK ID field.
MED-PAK ID: This option reprints a form for one patient if the file was created previously using Export to File. To use, YES must be entered in the Reprint Form field. Enter the MED-PAK ID using the following format: MP (for MED-PAK) Patient MED-PAK ID, date. Example: MPSMITHJOHN01012024
Box Date Range: Enter the date range to print on the MED-PAK form. If blank, nothing prints.
Alt Warning: Enter NO to print, "CAUTION: Federal Law Prohibits the transfer of this drug to any person other than the patient for whom it was prescribed." Enter YES to print "Warning: State or federal law prohibits the transfer of this drug to any person other than the person for whom it was prescribed."
Print Manuf: Enter YES to print the manufacturer of the drug.
6. Click Finish and the form prints.
