Add a New Patient Record

There are two ways to add a Patient Record. It is recommended to always search for a patient before adding them into the system to verify they are not already entered. One way to search is from New on the menu bar; the other way is from the Search icon from Rx Processing Tasks.

1. Access NRx.

2. Click Search on the Rx Processing Tasks menu. Select Rx/Patient.

3. Type one of the following to search for the patient:

4. Click Find or press Enter to search for the patient. The Patient Scan displays.

5. If the patient is not in the scan, click on the Vertical Tool Bar or press CTRL+N to add a patient.

7. Use the following definitions to add or edit information in each Patient Record/General Information field as needed:

Patient Info  
Last Name, First Name, MI Type the patient's first and last name and middle initial.
In Care Of Type the name of the person responsible for patient charges. This name prints on statements and invoices.
Title Select the patient's title from the drop-down list.
Full Name Check this box to  change the Last, First and MI fields to one Full Name field. This is typically used for business and nursing home accounts.
Address Type the patient's mailing address.
USPS Verify Click this link to verify the patient address through the United States Postal Service (USPS). This web service requires the QS/1 Web Services Gateway. Contact Customer Support to have the gateway set up on your system.
 
The results of the verification display next to the icon. If verified, a check displays; if not verified an X displays. If a verification request has not been made, an additional icon does not display. When a new patient is added, or changes are made to an address, the system prompts to send a verification request.
City, State City Type the patient's city. Select the state from the drop-down list. Type the zip code.
Phone (H) (W), Cell Phone, Alternate Phone Type patient's telephone numbers.
SSN Type patient's social security number.
Sex Select patient's sex from the drop-down list.
HIC ID Type the Health Insurance Claim ID (up to 20 characters) used for Medicare Part D.
Birth Date Type the patient's date of birth.
E-mail Type the patient's email address. Click the envelope icon to send an email.
Doctor Click the double arrows to display the Prescriber Scan. Select the patient's primary physician from the scan.
YTD Rx Amount System-generated entry. Total prescription dollars for the year.
Price Plan System automatically populates the Price Plan assigned to this patient.
Facility Information  
Facility Name Click the double arrows and select the facility for the patient from the Facility Scan.
Facility Code System automatically populates the Facility Code from the Facility Record.
Room # Type the room number assigned to the patient.
Place Of Service Select the Place of Service from the drop-down list. This field replaces the Location field for NCPDP D.0 claims. The Location field will remain to facilitate transmitting 5.1 claims during the transition from NCPDP 5.1 to D.0.
Medical Record Num: Required entry for nursing home patients. Type the Medical Record Number assigned to the patient.
Admit Date Required entry for nursing home patients. Type the date the patient was admitted to the nursing home.
Location (5.1 Only) Select the Location from the drop-down list. This field was replaced with the Place of Service field for NCPDP D.0 claims. This field will remain to facilitate transmitting 5.1 claims during the transition from NCPDP 5.1 to D.0.
Account Number Type the account number assigned to the patient by the facility.
Level Select the patient's required level of care from the drop-down list.
A/R Information  
Charge Account Click the double arrows and select the person responsible for patient charges from the Charge Accounts Scan. If the patient's bill is sent to an institution, type the Nursing Home Identifier Account Code in this field.
Account Limit Type the dollar limit for the patient's Charge Account. A warning displays during order entry and account posting when the account exceeds this amount. Type a default entry for this field in the Account Receivable Options.
Last Paid System automatically populates the last date a payment was posted to the patient's Charge Account.
Auto Post Select the charge option from the drop-down list.
Frozen Check this box to freeze the Charge Account.
Previous Balance, Charges, Payments and Current Balance System-generated entries.

8. Press CTRL+S or click . The icons on the Vertical Tool Bar become active and additional patient information can be entered.

To add an Alias to the Patient Record:

1. Click Add Alias to display the Update Alias window:

2. Type an Alias (code) for the patient. Adding a user-defined Alias eliminates bringing up the Patient Record Scan to locate the patient. When entering a new prescription, type the Alias in the Patient field and the patient information automatically populates on the New Prescription processing screen.

4. If the Alias typed in the above window is already in the system, the following window displays:

5. Press to assign the new Alias to the Patient Record. Press to type a different Alias.

6. Click .

The Alias for the patient displays in the Also Known As field on the Patient Chart.

 Add Patient Records for this patient's family members.
