The Patient Record Additional Information screen contains patient identifiers (medical record, driver's license, etc.), patient preferences (language, safety caps, Health Minder), admission account numbers and interface options.
To access Patient Additional Information:

Medical Record | Required entry for nursing home patients. If Medical Record is populated on the Patient Record/General screen, that entry populates this field. If not, type the Medical Record Number assigned to the patient. |
Place of Service | If Place of Service is populated on the Patient Record/General screen, that entry populates this field. If not, select the Place of Service from the drop-down list. This field replaces the Location field for NCPDP D.0 claims. The Location field will remain to facilitate transmitting 5.1 claims during the transition from NCPDP 5.1 to D.0. |
Last Rx | System-generated entry. Prescription number of the last prescription processed for the patient. |
Driver's License | Type the patient's driver's license number. |
Location (5.1 Only) | If Location is populated on the Patient Record/General screen, that entry populates this field, If not, select the Location from the drop-down list. This field was replaced with the Place of Service field for NCPDP D.0 claims. This field will remain to facilitate transmitting 5.1 claims during the transition from NCPDP 5.1 to D.0. |
Weight | Type the patient's weight. |
Status | Select the patient status from the drop-down list. |
Patient ID #1, #2, #3, #4 and Qualifier #1, #2, #3, #4 | Type the patient's identification number (i.e., SSN, Driver's License, State Issued ID, etc.) and select the defining Qualifier for the ID from the drop-down list. Up to four IDs can be added for a patient. |
Medicaid Indictor | Select the patient state from the drop-down list. |
Medicaid ID Number | Type the patient's Medicaid ID Number (up to 20 characters). |
Discount Schedule | If the patient is eligible for a discount, click the double arrows and select the Price Plan with the discount to apply. |
Tax Schedule | Click the double arrows and select the Tax Schedule to apply. |
Undeliverable Address | Field is used in the Refill Reminder Program. The default is unchecked. Check this field if a Refill Reminder Letter was returned and marked undeliverable. When the address is corrected, remove the check. |
Pregnant | Check this field if the patient is pregnant. |
Visually Impaired | Check this field if the patient is visually impaired. |
Edit Admission Account Numbers | Field for nursing home patients. Click Edit Admission Account Numbers to make any required changes. |

Medical Expense Summary | Select the patient's preference for receiving medical summaries from the drop-down list. |
HealthMinder Patient | Check this box if the patient is HealthMinder Patient. |
IVR Callback Number | Type the IVR telephone callback number for this patient. |
Language | Select the patient's language preference from the drop-down list. |
Safety Cap? | Check this box if the patient requested safety caps. |
Contact Method | Select the preferred contact method for the patient from the drop-down list. |

Cycle Group | Four-character, user-defined field used to identify patient participation in the Cycle Rx program |
Allow Profile Access | Check this option for head of household members to allow the head of household access to their prescription profile. The Patient Record/Additional Info screen displays the new field, Allow HoH Access, under Patient Preferences. If the patient does not have a head of household listed on the screen, this option is greyed out. |
Refill Reminder | Select the patient's preference for Refill Reminder from the drop-down list. |
Opt in Data Collection (PHI) | If checked, customer information is used in data polling participation. Default = Y (checked). This option refers to PHI not used for treatment payment or operations, and the patient does not receive marketing communication. The pharmacy does financially profit from rebates from the distribution of the PHI. Current rebate services are Freedom Data, IMS, Roche and Wolters Kluwer. Reconciliation services are not included in this service. |
Delivery Method | Options are Delivery, Mail or Pickup. |
Opt in Marketing (PHI) | If checked, customer receives marketing communications (mail, email, fax or phone) through the use of their PHI by a third party or the pharmacy. The pharmacy or third party receives financial compensation. Default = N (unchecked). This option is greyed out if the QS1AUTH option, Pharmacy First Formulary, is N. If the Opt in Marketing (PHI) is changed from unchecked to checked, a Verification screen displays the message that the patient signature has been received for participation and then prompts for the Pharmacist ID and Password. |
Head of Household | Click the arrows next to the field and the Patient Scan displays. Select the patient to add this Patient Record as the Head of Household. |
340B | Check this box for 340B. Default is unchecked. |
Compliance Pkg | Check the box to interface with SMARxT Caps. |

The HealthMinder Patient option is located in Patient
Preferences. Check this option to remind a patient that a prescription
is due to be refilled. If checked, the HealthMinder Rx option on the Drug
and Dosage Information screen and the Additional Information screen of
the Prescription Summary is automatically checked when filling a new prescription
in NRx.
