1. Access NRx.
2. Click Search on the Rx Processing Tasks menu. Select Rx/Patient.
3. Type one of the following to search for the patient:
4. Click Find or press Enter to search for the patient. Select the desired patient from the scan. The Patient Record displays.
5. Click on the horizontal tool bar and select Chart, or click on the vertical tool bar. The Patient Medication Therapy Review screen displays.
6. Use the following definitions to complete the information; click Save.

Sex | Select Animal, Female, or Male. |
Birth Date | Type the patient's birth date. |
Occupation | Type the patient's occupation. |
Date of Death | If applicable, type the date of death. |
Education | Select the patient's level of education from the drop-down list. |
Married | Check the box, if the patient is married. |
Ethnicity | Click the dropdown to access ethnicity values. This is a combination field that also allows free text.l |
Next of Kin | Click the link to access the Next of Kin window. Complete the name, address, city, state, zip, and phone fields. Each relationship description is linked to a relationship code. This field must be populated, in order to save the information. |
Misc. Identifier | Type user defined patient identifying information. |
Religion | Type the patients religious preference. |
Race | Click the dropdown to access race values. The default is blank. |
Primary Language | Type the patient's primary language. |
Mother's Maiden Name | Type the patient's mother's maiden name. |
Also Known As | Type the name the patient's goes by. |
Father's First Name | Type the patient's father's first name. |
Multiple Birth Indicator | This field can be populated with Yes/No. If Yes, the Brith Count field displays. |
Birth Count | This field only displays if the Multiple Birth Indicator is answered with Yes. |
Recorded Measurements | Complete these fields as necessary. |
